3. Eligibility

To use our services, you must:

  • Be at least 18 years of age or have parental/guardian consent
  • Reside in our service area
  • Meet the eligibility criteria for specific services (e.g., NDIS requirements)
  • Provide accurate and complete information
  • Comply with all applicable laws and regulations

4. Service Agreements

When you engage our services, you agree to:

  • Provide accurate and complete information about your healthcare needs
  • Cooperate with our healthcare professionals
  • Follow recommended care plans and treatment protocols
  • Notify us of any changes in your condition or circumstances
  • Pay for services as agreed in your service agreement
  • Treat our staff with respect and dignity

5. Privacy and Confidentiality

You agree to:

6. Payment Terms

Late payments may result in service suspension or termination.

7. Cancellation and Refund Policy

We understand that circumstances may require you to cancel or reschedule services. Our cancellation policy is as follows:

  • 24+ hours notice: No charge for cancellation
  • Less than 24 hours notice: 50% of service fee may apply
  • No show: Full service fee may apply

Refunds will be processed according to our refund policy and may take 5-10 business days.

8. Limitation of Liability

To the maximum extent permitted by law, Luce Healthcare shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to:

  • Loss of profits, data, or use
  • Business interruption
  • Personal injury or property damage
  • Emotional distress

Our total liability shall not exceed the amount paid for the specific service giving rise to the claim.

9. Indemnification

You agree to indemnify and hold harmless Luce Healthcare, its officers, directors, employees, and agents from and against any claims, damages, losses, or expenses arising from:

  • Your use of our services
  • Your violation of these terms
  • Your violation of any rights of another person
  • Any misrepresentation or false information provided by you
